We wish to thank many people for their invaluable
assistance with the Fifth Edition of The American Psychiatric
Publishing Textbook of Psychiatry. First, we are
grateful to the outstanding authors who produced exceptional
chapters and who labored to respond with
good humor to our many editorial suggestions and critiques
of their chapters. Also, our distinguished editorial
board worked closely with us in designing a clinically
focused and scientifically substantiated volume.
The outstanding staff at APPI has been highly encouraging
and effective with all aspects of this project.
Ron McMillen, CEO, and John McDuffie, Editorial Director,
have been perennially insightful and invaluable
to the concept, organization, and content of all editions
of the Textbook. Greg Kuny, Managing Editor, assisted
us with key structural elements of the book and coordinated
the entire production process. The Project Editor,
Rebecca Richters, had the daunting challenge of
overseeing the line-by-line editing of all of the manuscripts
and asking for additional information or clarification
from our authors. Special thanks go to Ann Eng,
Senior Editor, who designed and coordinated the layout
of the book, and to Judy Castagna, Manufacturing
Manager, for ensuring that the published book was a
high-quality product. The original cover artwork was
painted by a talented Northern California artist, Dr. Peter
Shahrokh. Thanks also go to Kathy Stein, the highly
competent Director of Financial and Business Operations,
and the always pleasant and helpful Bessie Jones,
Acquisitions Coordinator, who aided us with numerous
administrative and technical requirements for the
publication of this edition. Finally, Bob Pursell, Director
of Sales and Marketing, has organized an outstanding
program for publicizing our Textbook to the field
through various promotional efforts.
The organizational headquarters for this edition of
the Textbook of Psychiatry was located at the University
of California, Davis School of Medicine in Sacramento.
Carrie Stafford handled all the correspondence
and the majority of the calls to our authors and editorial
board members. Her dedication and commitment
to the publication of this edition are greatly appreciated.
Susan Mortensen assisted me greatly in completing
the final editing and review of the page proofs and
in assembling the international advisory board. Susan
also prepared the Study Guide question and answers,
and Dr. James Bourgeois and Narri Shahrokh were
outstanding co-authors who helped one of us (REH)
write and edit the Study Guide. I would also like to
thank Cristina and Alain Camu for allowing me to
work at their wonderful home in Porto Ercole (Grosseto),
Italy, where I reviewed the page proofs and
wrote the initial drafts of the study guide questions.
Finally, and of most importance, we acknowledge
the many people with psychiatric disorders, their families,
and those who have dedicated their lives to caring
for them through clinical service, research, and education.
These brave and exemplary people have
provided us with the inspiration, motivation, and
knowledge for the crafting of this textbook.
