The author discusses the following problems encountered in attempting to establish a milieu treatment program for acute psychiatric patients: tyring to set up a ward culture in the midst of frequent admissions and discharges; the patients' inability to handle the principles of democracy and permissiveness; the spreading of problems encountered by one patient group to another patient group; and the blurring of staff roles.
