FREDERIC T. ZIMMERMAN M. D.1, BESSIE B. BURGEMEISTER PH. D.1, , and TRACY J. PUTNAM M. D.1

1 The Department of Neurology, Columbia University College of Physicians and Surgeons; and the Neurological Institute of New York.
